Common causes we see
Back pain rarely has a single cause. In our clinic in Vadodara we most often see it linked to:
- Long hours of sitting — office work, studies, driving, and long two-wheeler rides
- Lifting or bending awkwardly, especially lifting while twisting
- A bulging disc (“slip disc”) pressing on a nerve, which can cause sciatica
- Lumbar spondylosis — normal age-related changes in the spine that can stiffen it
- Weak core and hip muscles, so the back takes more load than it should
- Pregnancy and the months after delivery, when posture and muscles change
How we treat back pain at Nisarg
1. A careful assessment first
Dr. Hemal starts by asking how your pain began, what makes it better or worse, and how it affects your day. She checks how your back moves, tests the strength and nerves in your legs, looks at your posture, and goes through any reports you bring. This tells us whether the pain is coming from muscles, joints, a disc or a nerve — and what to treat first.
2. Easing the pain
To calm a painful back we use hands-on soft tissue mobilisation, electrotherapy such as TENS and ultrasound, heat, and — where it suits the problem — lumbar traction. These make movement easier so the real work can begin.
3. Exercise that fixes the cause
The long-term answer is almost always the right exercise. We teach you gentle movements first, then build strength in your core, hips and back, and improve flexibility. Exercises are checked at every visit and adjusted as you improve.
4. Habits and home care
We show you how to sit, sleep, bend and lift in ways that protect your back, and give you a short home programme so recovery continues between visits.

What you can do at home today
- Keep moving gently. Short, easy walks are better than lying in bed all day.
- Change position often. If you sit for work, stand up and move every 30–45 minutes.
- Use warmth for a stiff or spasming back — a warm shower or hot water bag for 15–20 minutes.
- Lift with your knees and hips, keep the load close to your body, and avoid twisting while lifting.
- Sleep comfortably — on your side with a pillow between your knees, or on your back with a pillow under your knees.
See a doctor urgently if you have back pain with:
- Numbness around the groin, inner thighs or back passage
- New difficulty passing urine, or loss of bladder or bowel control
- Weakness in the legs that is getting worse
- Fever, unexplained weight loss, or a history of cancer
- Severe pain after a fall or accident

Frequently asked questions
Do I need an MRI before starting physiotherapy?
Usually not. Most back pain can be assessed and treated without a scan. If you already have an X-ray or MRI, bring it along. If anything in your assessment suggests a scan or a doctor’s opinion is needed, we will tell you.
Is physiotherapy safe for a slip disc?
Yes, when it is guided by a proper assessment. Many disc problems settle with the right mix of pain relief, gentle movement and graded exercise. If your symptoms point to something that needs a surgeon’s opinion, we will refer you.
Should I take complete bed rest for back pain?
Long bed rest usually slows recovery. A day or two of taking it easy is fine, but gentle walking and normal activity — within comfort — generally help back pain settle sooner.
How many sessions will I need?
It depends on how long you have had the pain and what is causing it. After your first assessment, Dr. Hemal will explain the plan and give you an idea of how many sessions to expect.
